INCIDENT As stated above, as a production manager, I had call back into questioned 10 technicians from the incident place that is from the production sites to collect data on the causes of the production machines disruption for 48 hours. The incident on 13 July, 2012, were; at 12.24pm,as the staff were operating the production of pass gloves, suddenly the glove producing machines had a bizarre noise that panic-stricken all of them on that site. As we accounted these machine were making junior-grade noises few months back, unknowingly it will end up breakout down completely that day.

CAUSE The cause of the machine breakdown delinquent to mechanical malfunction by our department that caused by uncomely constant attention of the glove machines . These machines needs maintenance because of heat up daily due to continuing keel production overnight. From the interview conducted, the data that I collected shows that there were no periodic maintenance of the machines. 3 tec hnicians said that this closing was to main! tain a low cost on their production due to gamy maintenance cost. Whereas the rest admitted that they did not tear down take it a serious matter even later on was instructed by me. and so because of this irresponsible employees, the maintenance was not taken into consideration at all.
